Research Interests:

A 17-year-old male with no cardiac symptoms was found to have severe left ventricular hypertrophy (LVH), bicuspid aortic valve (BAV), and critical aortic stenosis (AS). Multimodality imaging demonstrated marked myocardial hypertrophy with asymmetric septal thickening. Cardiac magnetic resonance (CMR) showed patchy mid-wall fibrosis, while histopathology revealed myocyte hypertrophy and interstitial fibrosis without evidence of amyloid deposition. Genetic testing identified a transthyretin (TTR) variant of uncertain significance (VUS). This case highlights the diagnostic challenges in differentiating primary cardiomyopathy from secondary hypertrophy due to pressure overload, and underscores the importance of cautious interpretation of incidental genetic findings in the absence of phenotypic correlation.
