Improving Guideline-Directed Management of Heart Failure Through a Multimodal Quality Improvement Initiative in a UK District General Hospital

Background: Heart failure (HF) management has evolved with the introduction of guideline-directed medical therapy including ACE inhibitors, beta-blockers, mineralocorticoid receptor antagonists, and SGLT2 inhibitors. Despite this, early management is often delivered by non-specialists, leading to variability in care. Aim: To identify gaps in clinician knowledge and local HF guidance, and implement an intervention to improve management and confidence. Methods: A three-cycle quality improvement project was conducted. Baseline knowledge was assessed using surveys. Interventions included colour-coded mnemonic-based HF management posters (“ABCD” approach) and structured teaching sessions. Post-intervention surveys evaluated utilisation and impact. Results: Twelve clinicians responded. 85% reported exposure to posters, 71% used them in practice, and 77% reported improved HF management. 100% of teaching attendees reported improved understanding. Feedback supported wider dissemination. Conclusion: Simple, accessible interventions combining visual aids and education significantly improve clinician confidence and self-reported HF management.
